Guest Liability Release

I acknowledge that these are uncertain times. The risks of COVID-19 are not well understood and there is controversy among the experts on how the virus can spread and difficultly in scientifically determining whether anyone has the virus at any moment in time.

In consideration for Third Avenue Spa providing medical and/or traditional aesthetic services to me, by signing below, I agree to accept all responsibility for the risk that I may contract COVID-19. I acknowledge that while Third Avenue Spa is taking my safety and that of its staff very seriously by employing new safety and sanitation initiatives, I understand and accept that Third Avenue Spa cannot guarantee that any of these measures will completely protect me from contracting COVID-19.

I agree that if I take any steps to make a claim for damages against Third Avenue Spa, its agents, employees, affiliates, directors, officers or any other parties arising out of my receipt of medical and/or traditional aesthetic services during my visit to Third Avenue Spa's facility, I shall be obligated to pay all legal fees and costs incurred as a result of such claim.

I acknowledge that I can go elsewhere to have my medical and/or traditional aesthetic services, and I acknowledge that Third Avenue Spa is not the only spa where I can have my medical and/or tradiational aesthetics. By signing this Guest Liability Release, I acknowledge that I am free to go to other spas that may not require my agreement to accept responsibility for contracting COVID-19 and I freely and voluntarily choose to have medical and/or traditional aesthetic services at Third Avenue Spa.
